I'm having trouble filling in my drink diary

For patients submitting an Alcohol Dependency (only) consultation, we require a drinks diary. Please ensure that all the fields in the drink diary are fully completed for each day, with as much information as possible. If you have not consumed alcohol on one or more days, please state this and write "0" into that days fields where appropriate.

Most alcoholic drinks now detail the number of units contained on their labels but an idea of typical Units of Alcohol per drink is detailed below. For further information on how many units are in a drink, please see the Alcohol Concern calculator (opens a new window to another website)

1 shot of spirits (40%, 25ml) = 1 unit
1 large shot of spirits (40%, 35ml) = 1.4 units
1 pint of lower-strength beer (3.6%) = 2 units
1 can of medium strength beer (4.2%, 500ml) = 2 units
1 pint of high-strength beer (5%) = 3 units
1 glass of wine (175ml) of 12% wine = 2.1 units
1 bottle (750ml) of 13.5% ABV wine = 10 units
